Your family physician and continuity of care
Family medicine in Ontario is undergoing changes with the goal of connecting all residents to a primary care provider. As part of this process, patients are expected to be attached to one family physician.
Use of walk-in clinics or physicians outside the practice can affect continuity of care and may limit our ability to provide comprehensive, coordinated medical care. You may visit hospitals or specialists by referral from your family doctor.
Some clinics, including obstetrical, psychology or pain clinics, may be provided by non-specialist general practitioners. Other clinics do not have your complete medical history. Your family physician may not know what happened at another clinic, which tests were ordered, or what treatment and referrals were arranged. Results generally return to the clinic that ordered them.
Preparing for your appointment
Patients are booked by appointment. We require 24 hours’ notice for cancelled or rescheduled appointments to avoid a no-show fee. Bring your OHIP health card to every visit and notify the front desk of changes to your phone number or address.
Book appointments for prescription renewals. If the physician wishes to discuss results, the clinic will call you for a follow-up appointment. We fax referrals on the day of your visit. If you have not heard from a specialist within one to two weeks, we can provide their contact number. We encourage all patients to have voicemail.
